Note: This form must be completed and a copy of the Dissertation/Thesis Abstract must be received by the School of Public Health two weeks prior to the defense date. Copies of the Dissertation/Thesis must also be circulated to the examining committee at that time. This form must be submitted to the Office of Academic Affairs in order to schedule your final defense.
Thesis/Dissertation Defense:
Upon successful completion of the defense, the student completes the for the Dissertation/Thesis Defense and Final Examination Report, securing all of the needed signatures. S/he then emails the SPH Office of Academic Affairs a copy of the Report and schedules an appointment as soon as possible to discuss the copying and binding of the final iteration of the dissertation. It is the student’s responsibility to ensure the original copy of the signed Report stays clean for the student must include it in the bound copies.
Students must format the thesis/dissertation according to the directions found in the LSUHSC School of Graduate Studies Dissertation and Thesis Guidelines, pages 4, 5.

Note: This Student Health Form must be submitted to the Office of Student Health Services for processing prior to the first day of class. Louisiana State Law requires submission of these immunization records prior to class attendance.
