The Louisiana Consortium for Health Transformation (CHT) is administered by the Louisiana State University Health Sciences Center–New Orleans School of Public Health. CHT is supported by a Cooperative Endeavour Agreement Between the Louisiana Department of Health and the School of Public Health (LaGov #2000220529).

The Cooperative Endeavor Agreement between LDH and SPH will augment the mission of each entity by leveraging the resources and skill sets of the parties with the goal of leading towards a transformation of health. The goal is that Louisiana not be ranked last in health – 50 No More. The process of working to transform health in Louisiana involves building an information base to identify opportunities and measure progress. Work plans for projects are currently under development and review for their attention to public health, health care and financing in Louisiana and their focus on Medicaid.
